Title: College "Brag Sheet" Recommendation

I need this for my new guidance counselor to assist her in the counselor reference form for college, she's new (old one quit..who I was friends with) and the new one doesn't know much about me.

Any suggestions on what I should add would be greatly appreciated, I'm pretty sure I remembered all of my clubs but I could be missing 1 or 2.
