Both, and neither.  It's always different.  Generally it's away.

Right foot always.  I take my left shoe off first, always, too.

When you brush your teeth, which side do you start on?

I start on the bottom-left side of my mouth and progress sideways, then to the back of my mouth, and then to the top.
